Sub agreements

To strengthen linkages and synergize efforts between facilities and communities, CHASS-SMT will carefully select and provide direct funding through subagreements to Community-Based Organizations (CBOs) who have a strong community presence, established expertise, and are capable of adhering to USAID reporting protocols.These CBOs will support the following key activities:

  • Support peer case managers that will serve as a link between patients, the health facility and community to ensure access and linkages to a range of HIV-related services such as home-based care, orphans and vulnerable children, nutritional counseling, and other social and psychosocial support.
  • Improve Antiretroviral Treatment adherence support for patients enrolled at different points of care at the health facilities (i.e. HIV, TB, PMTCT, Malnutrition, chronic diseases).
  • Increase the demand for health services, such as HIV, tuberculosis , malaria, FP/RH, nutrition, and hygiene among others. Using an evidence-based approach, demand generation messages will be developed to target communities and individuals to seek health services.
